Watcha — Watcha emergence delirium scale

What this tool is and how to interpret its evidence

The Watcha scale is a brief observational rating used during recovery from paediatric anaesthesia. It orders behaviour from calm or settled states through increasing restlessness and agitation. Although it is often discussed in emergence-delirium studies, the scale primarily measures observable arousal and agitation rather than the full delirium syndrome.

Bajwa and colleagues compared Watcha with PAED and other emergence ratings after general anaesthesia. Watcha was sensitive for conspicuous hyperactive behaviour but less specific than the more syndrome-focused PAED approach. Pain, fear, hunger, separation distress and normal developmental behaviour can all produce similar observations. The scale is particularly poorly suited to hypoactive presentations and should not be extrapolated to general paediatric intensive-care delirium.
